Individuals on the waiting list can access support from the Shropshire Autism Hub while they wait, including information, advice, courses, resources and peer support opportunities.

How to Access an Assessment

To be considered for an autism assessment, you must complete an Adult Autism Assessment Referral Form.

All completed referral forms should be emailed or posted to us for processing. Please get in touch with us if you would like us to post you a paper version of our referral form. 

Already Been Referred by Your GP?

If your GP has referred you for an autism assessment, we will to contact you to complete our referral form before your referral can be progressed.

If you have not heard from us since being referred, please contact the Shropshire Autism Hub so we can check the status of your referral and ensure we have all the information needed to move your referral forward.
